Symphysis pubis - Secondary cleft sign

Seen on STIR coronal images and symphyseal cleft injection

Primary cleft: Midline single physiological cleft in the symphysis pubis on contrast injection or MR imaging is normal

Secondary cleft: Usuallu seen in the inferolateral aspect of the pirmary cleft and usually continuous with the primary cleft

What it means: Groin injury, seen commonly in athletes. In all sports injury secondary cleft sign is an important indicator of groin injury. The common causes include Gilmore hernia, osteitis pubis, tendonitis or avulsion of adductor origin, or combination of these conditions
