Synovial osteochondromatosis

Intriduction:
Benign monarticular disorder
Proliferation and metaplastic transformation of synovium with multiple cartilaginous nodules
More common in men
30-50 years
Most common joint - knee. Other joints include elbow, hip, shoulder
Present with joint pain, swelling, limitation of motion
Nodules may break into joint space

Radiograph:
Pathognomonic multiple intraarticular calcified uniform nodules
Associated with joint effusion, marginal erosions, secondary degenerative changes

MR:
Nonmineralized areas are seen as intraarticular conglomerate mass, isointense to muscle on T1 and hyperintense on T2
Cartilagenous bodies show small areas of low signal on all pulse sequences
Ossified bodies show low signal (cortical bone) peripherally and high signal (marrow) centrally on T1

Treatment:
Surgical synovectomy. Recurrence rate 25%
