AP Approach:
Supine with internal rotation
Mark the medial aspect of the neck of femur just under femoral head
Locate femoral artery and mark it
Pass spinal needle directly onto the neck of femur
Oblique Approach:
Supine
Mark a point half way between trochanters and another point at head of the femur
Direct needle from intertrochanteric point towards femoral head point at an angle to meet the head (approx 60 to 90 degrees)
0.1 ml Gd in 20ml normal saline
Joint capacity - 8-20 ml.
